here, I assume it to be "mischia", "battaglia" or "confusione", whereas in other contexts it might be "cagnara", "casino", or others.
Perhaps, it could be loosely rendered as:
"Si fa largo attraverso/(attraversa) la confusione della battaglia armato unicamente del suo tomahawk/della sua ascia di guerra".
However, I rather like the following: "Si fa largo nella mischia armato unicamente del suo tomahawk"
The latter could apply to a native american, as well as to a fighter pilot...

According to Webster's Dictionary, "the act of intentionally mutilating a person's body of injuring it so as to deprive him of a limb or any organ of the body that he needs to defend or take care of himself;" Figuratively, mayhem can mean "macello", "caos". From the context given, however, I think that mahem refers to "macello" in its literal sense.
