opening up/cutting an opening

you open up something i.e. without context you may open the crown, the gums, whatever, you must know that from you context, this comes from brain surgery where you open up, by trepanation, a permanent opening.

Comes from trepan, according to Merriam Webster

Main Entry: 1tre·pan
Pronunciation: tri-'pan
Function: transitive verb
Inflected Form(s): tre·panned; tre·pan·ning
Etymology: Middle English, from trepane trephine
1 : to use a trephine on (the skull)
2 : to remove a disk or cylindrical core (as from metal for testing)
- trep·a·na·tion /"tre-p&-'nA-sh&n/ noun

I have heard it used in regard to the Aztecs. They practiced it. You remove a disk from the skull in order to get at the brain. From what I have seen, some of the patients of the procedure lived through it for some time--deduced from the bone growth around the hole in the skull.
