I must have translated a zillion of these and the wording is somewhat fluid but the basic format goes something like this:
"...Certifico que en el libro no. XXX tomo no. YYYY del archivo de esta oficialia en la hoja no. ZZZ del municipio de AAA se encuentra el acta no. 010101 de fecha dd/mm/aa
Which is why I haven't suggested such a literal translation:
…I certify that in book XXX of volume YYY of the files of this register office, folio no. ZZZ of AAAA municipality contains entry no. 0101010 dated mm/dd/yy…

It is recorded in minute No.
Instead of "minute," you can use "report" or other possibilities depending on context.

The registrar is usually saying something like "I certify that in book XXX, volume YYYY... blah blah… folio YYY contains entry number 0000"

'Found' is redundant - we say 'is recorded/entered'.
