т.е. «характерная черта, (какая именно?) а именно прогрессирование инвалидизации» или «характерная черта, заключающаяся в прогрессировании инвалидизации»

Перевод «выраженное прогрессирование инвалидизации» был бы возможен, если бы не было слова “feature”. Да и когда говорят о степени выраженности прогрессирования, то говорят не «salient disability progression», а «severe disability progression». Поэтому думаю, что здесь речь не о степени выраженности.

Пример употребления конструкции “the salient feature of”, где предлог of имеет значение «какой именно, который заключается в…»:
