Is "first language" in "if English is not your first language" translated to 第一语言 or 母语? Which one is more suitable? Or both are the same?

首选语言

母语 is mother tongue. I am only speculating here, but I think ‘first language’ implies a choice. In the US, for example, many patients grow up in a bilingual environment. To them, perhaps both English and Spanish/Chinese/Vietnamese/whatever are their mother tongues. So they need to identify which language is their first language for communication.
