Context:
Femenina de x años se encontraba en la playa nadando y al dar una brazada, súbitamente sintió un chasquito e intenso dolor en su hombro izquierdo que le impedía moverlo.
Su pareja que se encontraba con ella describe el signo de la charretera militar del hombro izquierdo, nos refiere que al tratar de sostener el brazo de la misma forma que se produjo, súbitamente se redujo espontaneamente.

I have not been able to find a common name for this sign other than its description as a shoulder that "looks square" or has a "squared off appearance". The French "Epaulet" sign, although correct, seems very rare as a name for this sign (see Taña and liz Refs).

En tu enlace también se habla de step-off deformities ("signo del hachazo"). La descripción del hombro en charretera es perfecta; ¿tal vez después de todo "epaulet sign" no sea tan raro?

Como cuadro clínico, la luxación recidivante de hombro presenta una deformidad típica de la
articulación subcoracoidea. El brazo queda fijo en ligera abducción con ligera rotación externa y
dirigido hacia arriba y adentro. El hombro se presenta aplanado en “charretera” y una depresión o
“hachazo” inferior. El acromion es anormalmente prominente debido al relieve de la cabeza por
delante. El codo está flexionado y el antebrazo se halla en rotación interna. El lesionado tuerce la
cabeza acercando el cuello hacia el hombro lesionado y tiene dolor.
