Intanto, ribadisco quanto scritto nella tua altra domanda per "dummy" come termine singolo ( http://tinyurl.com/nxztohg ), che ho inteso come testo segnaposto, ovvero come indicazione rispetto all'impaginazione finale del sondaggio.

Quanto a quel "punch", potrebbe riferirsi alla tipologia di raccolta dati, effettuata tramite dispositivo "punch (in/out)", che sembra essere una sorta di tastiera virtuale (tipo telecomando, pare) in cui inserire le informazioni dei partecipanti, suddivise in voci prestabilite, come dati anagrafici, geografici, ecc.
A titolo di esempio, ti segnalo l'estratto da un articolo del New York Times relativo a un'indagine Nielsen sulle predilezioni televisive, che mi ha dato lo spunto per il mio suggerimento:
"The instrument [a ''people meter,'' is a hand-held device] - as opposed to the currently used black boxes that monitor what is being watched in 1,700 Nielsen-surveyed households - is about the size of a remote-control channel changer. It has eight number buttons - one to be assigned to each family member, with the extras for visitors. Surveyed viewers are to feed their sex and age information into the people meter; they are supposed to punch in when they begin watching and punch out when they leave the room."

Se poi vuoi una proposta più azzardata, Punch potrebbe anche essere il nome dell'agenzia incaricata del sondaggio. Ad esempio ne ho trovato una, australiana, che si chiama PUNCH digital, e che si occupa di marketing digitale...
