Slovak to English Other Certificates, Diplomas, Licenses, CVs
From a birth certificate, under the number given in the top right-hand corner, as in:
Č.: dož. XXXX/XXXX
No other context - does anyone have any ideas?

I was going to ask if it was a copy. Then "na dožiadanie" might make sense.

No, it's unusual; I have 6 Slovak birth certificates here, and this only appears on one of them. Plus I have translated hundreds of Slovak birth certificates in the last few years and I've never seen it before. The person in question was born in 1992 but the certificate was re-issued in 2015, but that's not so unusual. It looks like what I've put above (with actual numbers instead of XXXX/XXXX, obviously)... not much help!

I do not see anything like that on birth certs of my family. How that number looks like?
