Yes, it is one of the hospital jobs. Actually I am wrong to say equivalent to a social worker, who needs properly professional training and an undergraduate degree.
This kind of job could be filled without a requirement of a degree, and mostly get trained on site. I should say they help patients out with proper info and refer the patients to the right places with right professionals for their specific needs.
Not sure how to translate into Chinese though.
