To Asker: Yes, I think so, from the context - what he's replying to and how he says it. Who else would he be talking about? You can add "I'm," but I think the speaker wouldn't. Also, it's more important to add "obviously" than "I'm." The reason is that, the way he says it, it's like he's saying: look at me and my behavior, and obviously you can see that I'm nuts. Actually, in the film doesn't he say "Ну псих, видишь"? "Obviously" creates the same effect as "видишь". Although, I suppose you could omit "obviously" and make it "So I'm a nutcase."

By the way, you asked about using the same word in the title. You could do that with, "Nutcase on the Sidewalk." Or, vary it to "Wild on the Sidewalk," echoing "Wild in the Streets," and don't worry about matching the title to that part of the dialogue.
