Spanish term

E. Peticionaria

Spanish to English Medical Medical (general) Pathological Anatomy Report
"Peticionaria" could refer to a feminine entity requesting something, but I don't know what the "E" stands for. This term appears at the head of a pathological anatomy report, with introductory text appearing in the following order:

Name of report & relevant report numbers

Prescribing Dr. __________ Date of Record
[Date]
_E. Peticionaria_
XXXX General Surgery I-Hospitalized

Could "E" stand for "Entidad"? I can't think of any other possibility.

Requesting nurse

It's hard to know without more context but "enfermera" fits for both the E and feminine adjective.
