They call a "rest period" a "period of rest", but that sounds a bit clumsy to me - but depends who the translation is for, I guess.

This is so close to everyone else's answers - here are my reasons for posting it separately: I think it's a statement signed by the employer separately from the control or logbook, to me "rest" or "rest period" corresponds better to driver's breaks than "time off" (which seems to me to indicate a longer period) and I don"t think "suggesting" is strong enough, as Cécile says we're basically talking about a lie here.
