form of oaffirmation: to/do solemnly and sincerely and truly declare.

2. affidavit: MAKE OATH AND SAY (note the upper casing)/SO SOLEMNLY AND SINCERELY DECLARE.
Can't find a deposition, but am sure it is the same. After all, the maker of an affidavit is called the deponent.
