Purpose of the authorization:

The *subject of the information* or Personal Representative initiated the authorization or coordinate care, facilitate billing, referral etc.

This is from a list explaining why an authorization for Release of Information is given.
How do you interpret "subject of the information"? Who do you think it is?

I also think the "or" in that sentence should have been "to". So the person, or someone who represents them initiated the process of authorization with the purpose of coordinating care, etc.
