In a tracheostomy patient notes section headed "Rondanteckning". LA. Ser blodprovsvar och pat. Planeras hem ..... Om pat kvar till [weekday] byter nog LA till okuffad track.
LA could mean a few things but not sure what it is here.

Sorry, I have no idea. Further down (another day in the notes) it says: LA: Pat forsatt träning med skötsel av stoma.
Next day: LA: Patient får gå hem.

This is little more than an eduated guess, but it seems that LA here could be the title of the person writing the notes, this job being based on the US role of physician assistant, with writing up notes, managing post-operative patients, changing tracheostomy tubes all being typical duties.
