Put polystyrene, an inflatable tube, whatever, in your formwork at the place where you want a void, then pour in the concrete.
Saves the trouble of (core) drilling a hole subsequently/

Are these actually "ducts" or are they "penetrations/ feedthroughs", i.e. holes through floors or walls for passing pipework, cables, etc.?

Can't be sure with the context, but sounds like the pipe used to inject the cement gout to fill whatever cavity.
