A letter to a bank, requesting transfer of funds from one account to another. The letter is sent by the Betreuering who works in an Alten- und Pflegeheim.

authorized/approved by the probate/surrogate court

Depending on the state in the US, the court granting such authorization and such guardianship/conservatorship is either called probate court or surrogate court. It is the same court that grants authorization to the guardian/conservator to transfer funds from one account to another.
In the UK it prob. falls under the jurisdiction of the High Court. They may call it guardianship court, but I am not sure.
The meaning of BetreuerIN in Germany is two-fold -
1. caregiver - just someone taking care of a child or an elder but a caregiver has no authority to touch bank accounts unless he or she is also (see number 2)
2. guardian/conservator - either ordered or appointed by the court
