I'm having a bit of trouble working out if this refers to housebound seniors (i.e too sick to leave the house unaided) or if it refers to seniors who still live at home (i.e. have not been admitted to aged care facilities).

In the absence of other context, I think in general it means "senior citizens who live at home."

Beyond this, they may also be:
1) Just senior citizens living at home. (healthy)
2) Senior citizens who live at home and receive special care from someone (special nurse, etc.) who comes over to the home.
