English to Spanish Medical Medical: Health Care

This is the name of a program offered by an insurance company in which dieticians and nurses work with at-risk patients over the phone about diet and exercise to try to lower cholesterol or blood sugar levels and/or lose weight to minimize risk of heart attack or diabetes or get off of medications. I think it should be something like 'Comprensión/Conciencia del bienestar para mejorar la salud'. What do you think?
