Os trigonum syndrome is a painful posterior impingement process. The patient will feel pain in the posterior ankle from the plantar flexion or from raising on the toes, as would be seen in flexor hallucis longus tenosynovitis. This pain results from recurrent forced plantar flexion and is associated with flexor hallucis longus tendonitis. The soft tissues in the posterior aspect of the foot are often inflamed and edematous.
