sourse language does not seem to be Czech, I suppose the birth certificate not to be issued in Morava but in Slovakia

Could you provide little more info? It should say something like Jmeno otce, prijmeni matky, atd.
Fara - Parish
Dekanstvi = Deanery
Kmotr = Godfather

U postednouns in diffrnt cases - parts of expressions, we need more :)

father mother godfather parsonage deanery

otce and matky is genitive, kmotru is dative and fara and dekanstvi are in nominative cases. Hope this helps you.

neutral Prokop Vantuch : I suppose the source text contained "kmotrů", which would indicate genitive as well, but in plural. on the other hand, without further context it's only a guessing game :-)
