d/c I.V.

In Latin American Spanish, "canalizar" refers to an I.V. catheter/cannula. In English, however, there is no single verb to refer to the removal of the catheter. The commonly used term is to "discontinue" the intravenous catheter. The short (and most common) way this is expressed is to "d/c" the I.V., but you can also just use "remove." In this context, it is clear that the patient's I.V. was discontinued and oral liquids were initiated.

I'd stay away from extubate, as this refers to tracheal intubation, not an I.V. catheter.
