This appears in a medical report in which a person with polyarthritis is given a number of tests, e.g. thumb opposition. I think that someone who is familiar with these different tests will immediately know what this is.

You put your right hand over your left shoulder (or vice versa) and then try to touch it with the other hand. Not sure what you'd call this, but I'm not sure "back stretch" covers it.
