maybe this diagram illustrating the steps in a clincial encounter will help

agree Lancashireman : Yes, the first 10 examples from a Google search clearly substantiate this phrase. Doctors have this very strange intransitive use of the verb ‘present’: “The patient presented with…”

chief complaint

The patient's reason for presenting to the clinician is usually referred to as the "Chief Complaint." Perhaps a less pejorative/more accurate nomenclature would be to identify this as their area of "Chief Concern."
