so simply put, the policy will continue even if if there is a change of provider, to one outside the network, of the healthcare for those patients already receiving care

agree Tony M : Yes, it does make sense, it is just a slightly unusual usage of 'follow' with 'to'. It is used with certain kinds of policies where the 'benefit' follows the person as they move to a different provider. Don't see much wrong with ' to transition members'?

Thanks Tony! Yes "transition members" is also unusual. We'd usually say "follow on" when it means continue or staying in place//not "wrong" per se. Just looks a bit strange to my eyes

"to transition" is an infinitive verb. "to" is not part of "followed", but is part of "transition".

It would be easier to understand if you put a comma after "followed":

Thank you! I also thought of "adhered to", but avoided it because that would be one "to" after another.

The member transition process helps ensure that members don’t have delay in services when they change health plans or providers. This change can be due to:
