The French title is "Inconnu à Cette Adresse".
Do read this book. It is a series of letters written in the 30's, bestseller then and bestseller again recently when republished by pocket books, Washington Square Press. A tiny book it will leave you breathless with admiration.
For your form you could always put: "oui", "non", "reponse inconnu". Then it's very clear.
