It's a trademarked scale/tool, "a measure of functional independence". You'll have to discuss it with the person/s who developed it and registered the trademark. Marie has provided clear evidence that "measure" is the tool's correct name.

Definition: An assessment of the severity of patient disability. ... The Functional Independence Measure (FIM™) instrument is a basic indicator of patient disability. FIM™ is used to track the changes in the functional ability of a patient during an episode of hospital rehabilitation care.

The text is a discharge slip from a hospital dealing in Geriatrics.
Under the Heading Functionality (Funcionalidad), there is a heading FIM under which there are two sub-headings FIM Ingreso and FIM egreso. There are 3 parameters measured under each Ingreso and Egreso:
