His examination includes "ESTADO GENERAL AL INGRESO, SIGNOS VITALES, NEUROLOGICO, ODS - PUPILAS ICNR, ORL - PALIDEZ GENERALIZADA, CUELLO, TORAX, CARDIACO, PULMONAR, ABDOMEN...."
I believe it must be a type of observation, together with "ODS", but I can't find it anywhere on the net.

ORL is the standard abbreviation for otorrinolaringología in Spanish. It's so common that it's difficult to see how this acronym could refer to anything else.

In English it is generally known as Otolaryngology, though the correct full name is otorhinolaryngology, and the standard acronym is ENT (for ear, nose and throat):
"Otolaryngology or ENT (ear, nose and throat) is the branch of medicine and surgery that specializes in the diagnosis and treatment of ear, nose, throat, and head and neck disorders."

I presume the pallor and the mention of the neck relate to observations of the face and neck during the ENT examination.

"ESTADO GENERAL AL INGRESO, SIGNOS VITALES, NEUROLOGICO, ODS (oido derecho supurante -otorrea-) - PUPILAS ICNR (isocóricas normoreflexicas), ORL - PALIDEZ GENERALIZADA, CUELLO, TORAX, CARDIACO, PULMONAR, ABDOMEN... This is clearly a resident note... Where nothing makes sense but over the local service.... The (physical) descriptions of clinical history findings start from head to toe... For some reason as Charles state (otorrinolaringología / OTO) here must be related to ears, eyes, nose, mouth and throat... well... MD jumps to skin, then neck and as (almost) all residents... a mix where you have to "search" carefully...
