This is the title of the report, you have "Boletín de Calificaciones" y "Boletín de Observaciones". Elementary School. It´s from Argentina, goes to the USA.

This sounds the most natural to me. At least from the point of view of someone from California. There is the Report Card and the Student Evaluation.

My experience at school was that the marks and the comments all went on one report form - but that was in the UK, not Argentina
