This appears several times in a series of hospital progress notes in a patient's medical records. I presume it means the doctor got a data entry clerk (?) to enter the information instead of doing it himself. Can someone confirm this and perhaps suggest a succinct way of expressing it?
This phrase also appears frequently: 代行承認済:[date, time] 責任者:医師)[name of doctor].
Would something like 'data entry approved' be sufficient for this?

代行入力 entry is a clerical level of work, so as you said, a hospital has certain personnel to do the job whether it is contracted out or not.
Then the doctor check (as a matter of procedure) if the entry is made correct, then approves when OK, thus 代行承認済 by 責任者, namely 医師.
