I can't tell if this is a tautology just reiterating the "dichiara di nominare e costituire", which can therefore be omitted, or if there is some way of rendering it in English

So far I have written
"The aforementioned Appearing Party, the personal identity of whom I, the notary, am certain, with this deed
HEREBY DECLARES
that he nominates and appoints as representative of the company " s.r.l.", Mr:..."
