Sole practitioner
The phrase "sole practitioner" is quite common. The sentence could be structured: "In the case of a sole practitioner..." (for example).
Just an alternative, check out Google hits ("sole practitioner" + "medical")
Examples:
"Steven M. Stoller, M.D., is the sole practitioner and surgeon exemplar of American Orthopedic and Sports Medicine, the premiere medical center for orthopedic care in Northern New Jersey. "
"So he leased additional space next to his office to create a walk-in clinic. As the sole practitioner, he divided his time between house calls and the clinic."
