Bei Diagnosestellung liegt die Resektionswahrscheinlichkeit nur bei 15% bis 20%. Die Rate von RO-Resektionen ist gering. Probleme bereiten vor allem der retroperitoneale Resektionsrand mit neuraler Infiltration und Lymphknotenmetastasen, die die Prognose der Patienten drastisch verschlechtern. Während die 5-Jahres-Überlebensraten bei Patienten mit RO-resezierten Tumoren mit 28% bis 35% noch akzeptabel sind, beträgt die Gesamt-5-Jahres-Überlebensrate der Patienten mit Pankreaskarzinom nur 0,4%.
