segundo departamento de cirugía
Para mí es "segundo departamento de cirugía", pero como si con eso quisiera diferenciarlo del departamento de cirugía en el que atendieron al paciente antes. O sea, en el 1º dep de cirugía, no le encontraron nada y, en el 2º dep de cirugía que visitó, le encontraron la fístula.
