If you designate a trust or estate, you must attach the pages that reflect the legal name of the trust or estate, the beneficiaries (names, addresses and SSNs) and the execution date of the trust or estate. If the designated trust or estate is a valid trust or estate after the employee's death, and is not otherwise considered revoked or superseded or terminated, the trust or estate will not be considered to have predeceased the employee. Please consult your trust attorney before designating your trust or estate as your beneficiary(ies).
