RELEASE AND APPROVAL OF ESTATE TRUSTEE ‘S PROPOSED DISTRIBUTION OF ESTATE ASSETS

I, , hereby acknowledge that I have received from XXX, Trustee of the Estate of the late John Nowak, a copy of the proposed distribution of the assets of the Estate of the late John Nowak.

I hereby accept and approve of all items appearing in the said summary including the compensation claimed by the Estate Trustee as set out therein.

It is hereby understood and agreed that in order to save the Estate the expense of an audit, I do hereby waive an audit of the estate accounts.
