This is the usual way of expressing it in classical Latin. In the first, 'Florianus' is in apposition with 'nomen'; in the second it is in apposition with 'mihi' (possessive dative).

: "Nomen" is a neuter noun, not masculine, so it should be "nomen meum".

Nomen meum Florianus est.
I think that's it. Nomen is a neuter noun, so the possessive adjective would be "meum" (as opposed to "meus" or "mea") to agree with it.
