"angeschnitten" As Anne clearly describes, see Susanne's reference below, "angeschnitten" has to do with finding something incidentally (here already nebenbefundlich), in this case the hepatic/liver cyst. The liver cyst was shown incidentally in the subcostal plane/view made as part of the transthoracic echocardiography. If I understand it rightly, there was no subcostal image of the liver cyst made. Normally a transthoracic echocardiogram comprises the parasternal, apical, subcostal, and suprasternal windows.

As I understand, the liver/hepatic cyst was found incidentally in the subcostal plane/view of the transthoracic echocardiogram.

Transthoracic echocardiogram

Evaluation of the heart with echocardiography requires having "acoustic windows" of the heart. Bone reflects the ultrasound waves and so all structures directly behind bone are not visible with ultrasound. This requires that the heart be viewed between bones and, in particular, between ribs. The most common views are the parasternal, apical, subcostal, and suprasternal windows.

Assuming the context of looking at an echocardiogram behind the heart and viewing the subcostal hepatic cyst.
