Hi there!
I added a note to my suggestion. We do not have much context, but I thought that perhaps we could change the sentence order and say something like: " "XX shall necessarily join/retain/purchase a/the/our health insurance plan."

Where "XX" stands for "employees", "parties" or whoever might be involved in this context.
Laura Hafner Sep 1, 2019:
@mark In that case, I would probably go with "enrollment". So yes, Teresa's suggestion comes closest.
Mark Pinto (asker) Sep 1, 2019:
@laura Thanks for the suggestion!
PS: there is no reference to any specific plan.
Laura Hafner Sep 1, 2019:
@mark No problem. If this indeed is a case of the company having a specific mandatory insurance plan, then just make sure to use membership in "the" plan ;)
Mark Pinto (asker) Sep 1, 2019:
@laura Yes, enrollment would also be suitable. However, I'm seemingly likely to go with "membership of a medical insurance plan"
Laura Hafner Sep 1, 2019:
@mark Hi Mark. Yes, as I noted in my answer, opting does seem rather odd for something mandatory, but this is the term I almost always see for employer health insurance policies (assuming this is the case here). Membership might work, but to me implies some kind of "club". Perhaps "enrollment in"?
Mark Pinto (asker) Sep 1, 2019:
@laura @teresa @airmalrpl Thanks Laura. However, I feel "opting" is not a suitable term to use for something that is "mandatory". Maybe "membership" suggested by Teresa is suitable here. Let me know what you think.

membership to the healthcare insurance plan is mandatory

Sugestão.

A frase original usa o artigo definido. Então, suponho que haja menção de um plano de saúde específico, não qualquer plano. Daí que minha sugestão é "the...". Se não houver menção a um plano específico, então, naturalmente, seria "a...".

Uma outra possibilidade seria reformular a frase e simplesmente dizer: "XX shall necessarily join/retain/purchase a/the/our health insurance plan."

Pelo que entendi, pede-se a filiação a um plano específico (o plano de saúde da empresa?).

Although I realize the irony of a company making it mandatory to "opt in" (making what Brazilians would call "livre e espontânea pressão"), this a term I frequently see for services offered by companies and that employees can, allegedly, choose whether to accept - and pay for - or not.

Yes, this is my suggestion.
Enrolment is normally only for education and courses. Sign up is of more general use.
