In Mexico, "póliza" often refers to an official stamp or seal (sello fiscal). The "coupon" cited is sometimes known as a "voucher/receipt/proof", depending on where you look. As Wilsonn points out, it is a record of the transaction.

: yes, it is a tax stamp. They used to be quite common in the USA, prior to the introduction of the income tax in 1913, which eliminated the need for most of them, with a few exceptions.

Spain had them too until recently, a nuisance, as you had to go to an "estanco" tobacco store to get them before they'd issue your ID card, driver's permit etc...

We have this in the USA too. It is a two part check. The top part is the actual check and the stub is where the detail is printed. Usually this has the date, invoice number, description, and amount. There can be more information depending on what your computer program will allow you to print. You can usually do a custom format with most software.
