- Receipt number

- Place of purchase
- 情况说明 (should be about the customer's medical condition)

- Notes

I am tempted to call it a "guaranty card" because 信誉 does have a slight implication of "guarantee", doesn't it??? However, the company is not GUARANTEEING anything.... hmm...???

Warranty Card; Credibility Card

I did a research online because I am curious about 信誉卡. I just learned it by reading your KudoZ question. It turned out to be a fancy name for "Warranty Card". I looked into some 信誉卡 and found the content is very similar to the standard warranty card we have normally seen. The problem is that such practice in China got so abused and brought lots of complaints. For example, someone even put one on a watermelon. A consumer was not happy with the watermelon after she bought it and decided to bring it back for return. The shop attendant refused her request by simply saying watermelon met the store’s standard. Such abusive use of 信誉卡 is only for helping with the sale but not really guarantee anything.

If you really want a fancy translation for it like the Chinese version so as to differentiate it from the standard “Warranty Card”, I would suggest use “Credibility Card”.

Grammar mistake found. Here is the corrected sentence:

"Customer Information Card" or "Customer Information Sheet"

With those info you mentioned, I would think it is likely to a sheet, in stead of a card.

As it was not GUARANTEEING anything, it would be highly inpropriate to call it a guarranty card.

By sticking to “information card/sheet” and the info on it, the manufacturer would NOT be LEGALLY liable to anything, although it would usually choose to do something to make their customers happy.

By translating \"信誉卡\" into anything with a word such as “guaranty” or “warranty”, you – the translator – could be in hot water later. If the manufacturer is held liable in a court case brought by its customer, the manufacturer may later turn to you for its loss.

Answer to Denyce’s supplementary question:

When the manufacturer calls it a 信誉卡, the manufacturer is saying:

Look, if you have any problem with our product, we would help you. If we don’t, it would damage our reputation (信誉). We are putting our reputation (信誉) on the line. Trust us! OK?
