English to French Other Law (general) Loss prevention

I am translating a company policy regarding new standards of loss prevention. Employees have to read it and sign the document at the bottom. So, at the very end, there is a part that says "ACKNOWLEDGEMENT FORM". It is basically a part where it is written " I certify that I have read and understood this document..."

How could I translate ACKNOWLEDGEMENT FORM in French.

It is definitely not "accusé de réception", which is mostly used when you certify having received a document.
I thought about : "Confirmation d'acception", "confirmation de reconnaissance".

What would be the correct way to say this in French
