mit der Eintragung im Geburtenbuch einverstanden

Just a bit unsure here of what this refers to. The context is a confirmation of paternity certificate. The parents details eg. name, address, nationality etc are listed on a table in different rows. The next row in the table then, is split in two (a left and right column) with the left column containing "Religionszugehoerigkeit: roem. kath."
and the right column containing "mit der Eintragung im Geburtenbuch des Kindes [X] einverstanden [ ] nicht einverstanden". Does this mean that the religion entered i.e. roem, kath concurs with that entered on the child's Geburtenbuch? Or does it refer to something else entirely?
Any help/ suggestions will be most appreciated. Thanks :-)

It sounds as if it's indicating whether or not the father agrees to his religion being entered in the Geburtenbuch - i.e. it's optional. The details in the Geburtenbuch (register of a birth) form the basis of the Geburtsurkunde (birth certificate): the Wikipedia description of a Geburtsurkunde says:

"Heutige Geburtsurkunden enthalten folgende Angaben (§ 59 PStRG):

den Namen des Standesamts,
die laufende Nummer des Geburtseintrags zusammen mit dem Jahr der Geburt,
alle Vornamen des Kindes,
den Nachnamen des Kindes,
das Geschlecht des Kindes,
das Geburtsdatum des Kindes,
den Geburtsort des Kindes,
von den „rechtlichen Eltern“
- den vollständigen Namen,
- die Religion, * wenn dieser Eintragung nicht widersprochen wird*..."
