Use Koshelev

You cannt be sure, so use the direct transliteration when addressing the person: `Koshelev' will be perfectly OK.

I mean sometimes nobody is sure if it's `e' or `ё', so you'd better use `e'.

Some Russian second names are not so obvious about where the accent is. The others like my own are pretty obvious: Semyonov, despite I transliterate it as "Semenov"

Yes, in real life I would pronounce it like this. But again... I had a friend whose name was Matsnev, and I thought it was Мацнев, but later when we met in reality it happened he was Мацнёв.

agree Olga Vlasova : I went to school with a girl whose name was КОстылева. All teachers read it as КостылЁва, she always had to correct them. So you can never be sure having only a written text for reference. Even though it's a bit of a stretch, "e" could go either way.

Кошелев = Koshelev, Кошелёв = Koshelyov

Those names are slightly different, unless I'm missing something.

agree Anton Konashenok : Right, and the former one is more frequent than the latter
