The form is for a medical claim and was submitted to the patient's insurance.

I agree that this is not an admission slip since it has the dates the patient stayed in the hospital.

hospitalization certificate

Since it has the check-out date, it can't have been for admissions because they don't know when you get there how long you're going to stay. "Certificate of hospitalization" would work also. It's more formal. Depends on the context, I guess. A doctor would write "certificate of hospitalization" and his receptionist would say that, but the patient and other non-medical personnel would just say "hospitalization certificate" because it's simpler.

Ex:
Follow-Up Doctor's Receptionist: Well, sir, we need you to sign this release so we can get X Medical to send over your certificate of hospitalization.
Patient: Oh, my hospitalization certificate? Well, they gave me this copy for you before I left.

neutral Josephine79 : I think this is wrong for the same reason that the "admission slip" was, but in reverse. It refers necessarily to the end date of the stay, whereas when someone is in for a long time a "bulletin" can be issued to prove that they are in hospital.

In the case of a M+CO being audited for CHF, they will need to furnish at a minimum an actual **cover sheet from the medical record as proof of the hospitalization**. CMS may request additional supporting records if the cover sheet fails to confirm the CHF inpatient discharge diagnosis. A list querying their system for a CHF diagnosis will not suffice as proof of hospitalization.
