Assessment and Treatment of Fetal Alcohol Syndrome in Children and Adolescents

This paper describes characteristics of fetal alcohol syndrome (FAS) from infancy through adolescence. This paper also provides information regarding comorbidity of FAS with other psychological syndromes. The literature regarding assessment of FAS across childhood and adolescence is detailed; recommendations toward accurate assessment are included. Finally, this paper discusses treatment of FAS symptoms in children and adolescents.
INTRODUCTION
Since the first reported documentation of fetal alcohol syndrome (FAS; Jones and Smith, 1973), literally hundreds of research and review articles have been published on the syndrome. One possible reason for this high interest level is that FAS is a totally preventable syndrome (Rosett et al., 1980). Unfortunately, as many as 30–35% of pregnant women drink alcohol on a regular basis during the course of their pregnancy (Rosenthal, 1990). The incidence of FAS is approximately .29 – .48 per 1000 live births (Mattson and
Riley, 1998). Incidence rates have been found to be significantly associated with economic class and ethnic minority status, with higher rates in lower economic and minority status groups as compared with higher income and ethnic majority status groups.
CHARACTERISTICS OF FAS
The diagnosis of FAS is made when a history of prenatal alcohol exposure is confirmed and the individual meets the following three criteria: (1) pre- or postnatal growth retardation, (2) central nervous system abnormalities, and (3) craniofacial dysmorphology (Sokol and Clarren, 1989). A report prepared by the Institute of Medicine (1996) suggested additional diagnostic criteria for FAS, including brain structure deficits and small cranial size at birth. Other physical characteristics that have been found to be associated with FAS include eye, heart, ear, skeletal, and spinal abnormalities.
Research over the last decade has elucidated developmental variability in the characteristics of FAS. For example, although lower than average weight and height are extremely common in children with FAS, female adolescents and adults with FAS frequently maintain weight that is within the normal range (Spohr, 1996). In childhood, craniofacial dysmorphology may include microcephaly, short eye slits, a flattened mid-face, an upturned nose, a thin upper lip, and an indistinct philitrum. In adolescents and adults, craniofacial
dysmorphology most often includes microcephaly, a large nose, an
indistinct philitrum, a thin upper lip, and a pronounced chin (Spohr, 1996; Streissguth, Aase et al., 1991).

Este artículo describe las características del síndrome de alcoholismo fetal (SAF) desde la infancia hasta la adolescencia. Además, otorga información de comorbilidad en el SAF junto a otros síndromes sicológicos.
La literatura es detallada en cuanto a la evaluación del SAF en el periodo de infancia y la adolescencia; por lo tanto se incluyen recomendaciones hacia una evaluación precisa. Para finalizar, el artículo discute el tratamiento de los síntomas del SAF en niños y adolescentes.
INTRODUCCIÓN
Desde el momento en que se publicaron los primeros documentos acerca del síndrome de alcoholismo fetal (FAS; Jones y Smith, 1973), se han realizado cientos de investigaciones y artículos acerca de este síndrome. Una razón posible de tan alto interés es que el síndrome es completamente prevenible (Rosett et al.,1980). Desafortunadamente un 30 a 35% de mujeres embarazadas beben alcohol de forma regular durante sus embarazos (Rosenthal, 1990). La incidencia de SAF es aproximadamente de un .29 a .48 por cada mil nacimientos vivos (Mattson y Riley, 1998). La tasa de incidencia se ve asociada directamente a clase económica y minorías étnicas, obteniéndose un mayor índice en el grupo de bajo nivel económico y grupos étnicos minoritarios, al compararse los datos con sectores de mayor ingreso y grupos de mayorías étnicas.
CARACTERÍSTICAS DEL SAF
El diagnóstico de SAF se puede realizar cuando se confirma un historial de exposición prenatal al alcohol y el individuo además presenta los siguientes hechos clínicos: (1) retraso del crecimiento pre o postnatal, (2) anormalidades en el sistema nervioso central y (3) dismorfología craneofacial (Sokol y Clarren, 1989). Un informe preparado por el Institute of Medicine (1996) sugirió criterios adicionales para el diagnóstico de SAF que incluyen: deficiencias en la estructura cerebral y tamaño craneal reducido en el nacimiento. Otras características físicas que se encuentran asociadas al SAF incluyen anormalidades en ojos, corazón, oído, esqueleto y columna.
Las investigaciones en la última década han develado una variabilidad en el desarrollo de las características del SAF. Por ejemplo, es muy común que los niños con SAF padezcan de un peso y estatura bajo al promedio, las mujeres adolescentes y adultas con SAF frecuentemente mantienen su peso en un rango de normalidad. (Spohr, 1996).
Durante la niñez, la dismorfología craneofacial, puede incluir microcefalia, párpados cortos, rostro achatado, una nariz respingada, labio superior delgado y una depresión infranasal indistinguible. En los adolescentes y adultos se manifiesta una dismorfología craneofacial que a menudo incluye microcefalia, nariz alargada, ausencia de depresión infranasal, labio superior delgado y mentón pronunciado (Spohr, 1996; Streissguth, Aase et al., 1991).
