This is taken from a x-ray request form for a patient suffering from a fever with acute myeloid leukemia: "Quesito diagnostico: controllo in COVID". Another request form from a few days later states "Quesito diagnostico : Peggioramento sintomatologia respiratoria in paziente COVID-19 positivo"
Does this mean that they were monitoring for COVID-19 initially before it was confirmed with a test a few days later?

Normally "quesito diagnostico" is "provisional diagnosis" - a test is not a diagnosis.
The test in question is an X-ray, so not a "test for COVID" per se - they're obviously doing the X-ray because the patient is suspected of having C-19.
You have two choices here - either translate it as if it was a provisional diagnosis and go for "suspected COVID-19", or translate literally and go with Joseph's suggestion of "COVID-19 assessment", or the wordier "monitoring of patient with suspected COVID-19".

To my understanding the patient has already been diagnosed. This is a known case of COVID, they are monitoring the lungs.
