German term
Einzelpraxis
German to English
Medical
Medical: Health Care
In einer Einzelpraxis ist eine weitere Modifikation insoweit erforderlich, dass Sie den Nachnamen XXXX durch Ihren Namen ersetzen. Dies ist notwendig, da der eingetragene Benutzer auf allen getätigten Ausdrucken als die untersuchende und verifizierende Person erscheint.
This is about a user manual of a medical instrument designed for testing cardiac parameters. Would Einzelpraxis be translated as private practice? Surgery? Any suggesions?

individual private practice
I would put 'individual' in as well, since a private practice could also be a team of cardiologists, for example, not just a single doctor.

Sole practitioner
As the first answerer states, a private practice can follow several different business models.
The phrase "sole practitioner" is quite common. The sentence could be structured: "In the case of a sole practitioner..." (for example).
Just an alternative, check out Google hits ("sole practitioner" + "medical")
Examples:
"Steven M. Stoller, M.D., is the sole practitioner and surgeon exemplar of American Orthopedic and Sports Medicine, the premiere medical center for orthopedic care in Northern New Jersey. "
"So he leased additional space next to his office to create a walk-in clinic. As the sole practitioner, he divided his time between house calls and the clinic."
